high school transition services

This program, (Pre-Employment Transition Services) offers high school Sophomores, Juniors, and Seniors a comprehensive and empowering curriculum designed to prepare them for life after high school. Students will build essential life skills, improve their communication and interpersonal abilities, and gain hands-on experience in job readiness and workplace expectations. The program includes career exploration, personality assessments, and coping strategies to help students identify their strengths and interests, guiding them toward meaningful career options. Additionally, students will learn effective transition strategies to smoothly move from high school into the workforce, trade schools, or college. Based on the five core studies outlined by the Department of Education for Transition Services, this program aims to promote independence, confidence, and a clear path to future success.

To participate, students must have one of the following: 

          -A 504 Plan 

          -An IEP

          -A documented medical diagnosis or any condition that may impact their learning or                     participation.

As part of the program, staff from A Heart That Heals will work with students to identify employment opportunities that align with their skills and interests, both within the school and in the broader community. Students will earn at least the current minimum wage through A Heart That Heals. These paid work experiences are designed to foster skill development and independence, while ensuring that income is reported accurately to avoid affecting SSI or SSDI benefits.
